Limited Warranty

Chairside Shade Guide 8.0 ceramic tabs are protected under warranty for one (1) year for in the event that one or more tab(s) become broken. Should a ceramic tab become broken it can be replaced by contacting LSK121 Oral Prosthetics at 888.405.1238 or mail@LSK121.com. If a tab becomes broken within one (1) year of purchase the tab will be replaced at no cost, after one year the replacement will cost $60 per tab. The Chairside Shade Guide 8.0 base unit which holds the ceramic tabs is under warranty for one (1) year in the event that it becomes broken. Should a Chairside Shade Guide 8.0 base unit become broken it can be replaced by contacting LSK121 Oral Prosthetics at 888.405.1238 or mail@LSK121.com.

Return policy

There are no returns on Chairside Shade Guide 8.0. In the event that your Chairside Shade Guide 8.0 is shipped damaged, you have 30 days to return your shade guide to for a replacement. If your Chairside Shade Guide 8.0 was shipped damaged, contact LSK121 Oral Prosthetics at 630.955.1010 or mail@LSK121.com to request your replacement shade guide.
