1. Informational Purpose Only — No Referral
LSK121 Oral Prosthetics, Inc. (“LSK121”) is a dental laboratory, not a dental care provider. Any information provided by LSK121 regarding licensed dentists or dental offices is offered solely for general informational purposes.
Such information does not constitute a referral, recommendation, endorsement, or direction of care under Illinois law or any applicable federal regulation.
Patients are solely responsible for selecting their own dental provider.
2. No Practice of Dentistry
LSK121 does not:
- Diagnose or treat patients
- Create or control treatment plans
- Provide medical or dental advice
- Interact with patients for clinical decision-making
All diagnosis, treatment planning, and clinical services are performed exclusively by the licensed treating dentist, who maintains full professional responsibility under the Illinois Dental Practice Act.
3. No Legal or Clinical Liability
LSK121 expressly disclaims any and all responsibility or liability for:
- Dental or medical treatment decisions
Clinical outcomes or complications
Professional conduct of any dentist or dental office
Patient dissatisfaction, injury, or adverse outcomes
Compliance with state or federal healthcare laws
The treating dentist bears sole and independent responsibility for patient care and legal compliance.
4. No Financial Relationship or Incentive
LSK121:
- Receives no referral fees, commissions, kickbacks, or incentives
- Has no financial interest in patient treatment decisions
- Does not participate in billing, insurance processing, payment collection, or refunds
All financial arrangements are strictly between the patient and the treating dental provider.
5. No Agency, Partnership, or Control
Nothing in this notice shall be interpreted as creating:
- A referral relationship
- A partnership, joint venture, or employment relationship
- An agency or supervisory role
Each dentist operates as an independent licensed professional.
6. Patient Informed Acknowledgment
By accessing this website, application, or information, the patient acknowledges and agrees that:
- The selection of any dental provider is made independently and voluntarily
- LSK121 does not influence or control treatment decisions
- LSK121 shall not be held liable for any dispute, claim, loss, or damages arising from dental care or financial arrangements
LSK121 Oral Prosthetics reserves the right to share your information with third-parties who may be related to the services you are inquiring about.
Neither Luke Kahng nor LSK121 Oral Prosthetics are specifically reccomending any particular dentists who's information we share with ths tool. We are simply providing information to dentists who are currently using our lab for all or some of our cases. We cannot guarantee that the densits on this will use our lab for your case(s); and reccomend that you discuss the lab your dentist will use directly.
